Participant Registration Form

 

Programs and activities are specifically designed to benefit those in our community aged 55 or above.  All participants are required to complete the participant application form. 

 

If you've never been here before, you can save time by clicking here to complete the Participant Registration Form ahead of time.  Feel free to complete it and send it in or drop it off in person.  Once we have this information you will receive a swipe card that fits on your key chain.  You must use this swipe card to check in on the My-Senior-Center computer each time you are at Harleysville Senior Center.

 

Membership is optional.  The membership year runs July 1 - June 30 each year.  Membership is a $10.00 annual fee* and includes the following 2009 - 2010 benefits:
 

  • Our newsletter, THE FLYER, mailed to your home six(6) times per year

  • Voting rights at Member's Council Meetings

  • Free Program Member Status at Indian Valley YMCA (a $50.00 annual value)

  • 10% off Any Service (excluding oil change) at Just Cruisin' Auto Service Center

  • 10% off Good Neighbor Pharmacy Products at Rann Pharmacy

  • $5.00 off A Purchase of $50.00 or more at Harleysville Ace Hardware

* add $2.00 for each eligible additional household member
